Under CentOS-5 evolution, the plugin does not appear under new plugins,
from which I infer that it has no required configurable options.

(N.B. I don't have evolution handy, so the following menu locations are
untested.)
A new menu choice, MAPI, appeared under [Edit/Settings/Accounts/Add...]
for Server Type: .  I was able to select MAPI instead of Exchange
and upon doing so, dialog labels changed.  In particular, [Authenticate]
made debugging the identification of MAPI server much easier.
