FWIW:
I found an OCR app that is actually rather good: Text-R Pro. It's quite
reasonably accurate in conversion to text, and also preserves the layout
[almost] exactly. (I say 'almost' to account for any slight
inaccuracies!) -- so far. I don't think it's supposed to be free, but I
found a link that gives me a code to download an un-locked version (I'm
sure you all are familiar with this), so it cost me nothing. Yes there
are the occasional and expected conversion errors, especially near folds
in the original paper. But otherwise it's fairly accurate.

Background: I have years' worth of financial documents that is
cluttering my file cabinet, and I'm running out of space - I need to
clear it out. So I need something that gets me as close as possible to
the original without taking too much time to correct. Text-R Pro is the
best I've seen, and I'm not unhappy with it -- so far.

Anyone know of a good OCR app for Windows for converting a pre-existing
image file (e.g. JPG, etc.)? ... preferably either free or open-source?

I've tried a number of 'supposedly excellent' apps, but they either
don't give ANY clue how to import the image file, or they keep the
layout relatively intact but are REALLY poor at the basic function of
OCR, or they scan near perfectly, but scatter the info ALL OVER THE
PAGE. Think of a couple dozen numbers - all scanned perfectly, but each
digit is on a separate line and the lines are randomly placed !! It
would be far less time-consuming to simply enter the info by hand, than
to sort out that mess.
