spamassassin, amavisd-new, & dnsbl via spamhaus.
At work we block about 1500+ emails a day with the dnsbl from spamhaus alone.
And we tag about another 150+ messages that get filtered into users
junk mail folders.
To date we have not had a false positive, spamassassin is set to tag
at a score of 6 or higher.
