Nope, it will not consider the filename.
As a quick example,
kryptobsd:~ mstanislav$ echo foo>foo
kryptobsd:~ mstanislav$ echo foo>bar
kryptobsd:~ mstanislav$ md5 foo bar
MD5 (foo) = d3b07384d113edec49eaa6238ad5ff00
MD5 (bar) = d3b07384d113edec49eaa6238ad5ff00
