[mdlug] rc Scripts Not Running?!

I am going insane with this one.

I have a server (RHEL 5) on which I created a script in /etc/init.d to start and stop an application.  I modeled it so that it works with chkconfig.  I tested it from command line, it works fine.  So I went ahead and chkconfig --add'd it.  The symlinks were created  - S99app in /etc/rc3.d, rc4.d, and rc5.d.  The K25 scripts were created in /etc/rc0.d, rc1.d, and rc6.d.  The scripts run fine on boot and the app starts.  When I do a reboot, in looking at the console, it looks like the script is not being called.  What gives?  Anybody have any idea why the K25 script would not be called on reboot when it exists in /etc/rc6.d?  Unless I am horribly mistaken about how the startup/shutdown procedure works, I should see the script being run on the console just like the other shutdown scripts for nfs, vmware-tools, sshd, etc, right?
