I've been popping USB devices in and out lately (got a flash drive and
a USB Bluetooth transceiver for xmas) and I've run into that at times as
well. So far if I've unplugged the device, waited ~60 seconds, and
reinstered it, things have worked after that.
Depending on how your phone is implemented, it's possible that it's a
bit confused. I've seen a few problems (like my niece's phone refusing
to connect to a Bluetooth headset) be cured by popping out the battery
and forcing a full system reset on the phone.
